Boston, Massachusetts: Japan struggles for way to stop sex crimes against children

After finding marks on her children, a mother suspected the worst about a male baby sitter listed on a website. Whenever she needed someone to take care of her sons, she avoided the name Yuji Motte.

But her two young boys ended up in the care of Motte, a suspected pedophile who pretended to be a woman on the site. The body of one son was discovered in Motte’s apartment in Fujimi, Saitama Prefecture, in March 2014. The younger son, deprived of food, suffered from health problems.

At his trial, the court heard that Motte had molested at least 20 children.

But even if Motte had wanted help to prevent his pedophilia from leading to crimes against children, he would have few places to turn to in Japan, according to experts.

“Japan’s treatment system is lagging about 30 years behind, compared with the United States and countries in Europe,” said Hiroki Fukui, a psychiatrist who heads the nonprofit organization Sex Offenders Medical Center (SOMEC).

Experts say Japan lacks specialists on pedophilia who can form a network to prevent such crimes. They also cite the limited number of medical institutions that can treat offenders.

In response to calls to keep children safe from pedophiles, the Justice Ministry began a rehabilitation program in fiscal 2006 for prisoners convicted of sex crimes and offenders on probation. But Japan is ill-equipped to deal with such sexual predators before they can harm children.

The Yokohama District Court in July sentenced Motte, 28, to 26 years in prison for killing the 2-year-old boy.

According to the ruling, Motte, with the help of an accomplice, abducted the two brothers for several days with the intention of sexually assaulting them. He molested the 2-year-old before smothering him.

The boy’s brother, who was 9 months old at the time, went for hours without being fed.

The court also acknowledged Motte had sexually abused 20 children, including those aged from 4 months old to 5 years old. He had taken pictures of some of the victims’ genitalia. Other children were tied with string around their abdomens.

“The crimes he committed are extremely heinous in that he routinely committed sexually offenses against children,” Presiding Judge Takao Katayama said in his ruling. “The fact that he denies having pedophilia raises concerns that he will repeat his sex crimes.”

Motte appealed the ruling in August.

A psychiatrist who examined Motte told the court that the suspect indeed had pedophilia, a condition in which the person has an intense sexual attraction to prepubescent children.

“It would be preferable if he continues to be treated throughout his life,” the psychiatrist said. “He needs to check his sexual orientation on a regular basis.”

The Justice Ministry’s rehabilitation program centers on cognitive-behavioral therapy and is aimed at making sexual predators realize the “distortions” in their desires and understand the consequences for the victims.

Program participants are also asked to draw up action plans on, for example, how to control their urges.

So far, more than 4,500 offenders have taken the program, according to the ministry.

Many nations in the 1980s tightened penalties against sex offenders as a deterrent. But it became clear that simply locking up the assailants had little effect on preventing recidivism after their release.

Japan’s penal system focuses on rehabilitation and helping convicts assimilate into society through affordable therapies and other assistance after they serve their sentences. But it is not really designed for pedophiles.

At SOMEC, about 300 perpetrators of sex crimes gather monthly for cognitive-behavioral therapy in Tokyo, Osaka and Fukuoka. They pay for their treatment, and some are prescribed medication to reduce their sexual desires.

Fukui of SOMEC said a sweeping overhaul is needed for Japan to catch up with other nations in treating sex offenders to prevent crimes against children.

“Japan is still at the stage where it is trying to determine what kind of a setup should be in place to rehabilitate sex offenders,” Fukui said. “Right now, recipients are hard-pressed to pay for their treatment. The existing public health insurance system should be overhauled to offer affordable care.”

Chamblee, Georgia: Growing up in a prison filled with drug lords - The brutal Bolivian jail where families live alongside their criminal relatives

Eye-opening images have revealed what life is like for children inside Bolivia's notorious San Pedro Prison where the families of inmates live alongside drug lords. Rows of laundry hang across the brightly-coloured courtyard as children play with toy trucks below and murders, robbers and rapists lurk nearby. San Pedro prison, the biggest in Bolivia's main city, La Paz, is believed to hold around 3,000 dangerous inmates. The premise is the children and partners of the inmates are safer inside the prison than on the impoverished streets outside.

But they often fall victim inside too. In 2013 it was reported a 12-year-old girl fell pregnant after being raped by a group of men inside the prison. These offences do not go unpunished, with rapists and child molesters treated with a brutal zero-tolerance policy by the inmates 'council'. Being stabbed is one of the most common punishments. From the outside, San Pedro looks like any other jail but the inside is like no other prison on earth. There are no guards or metal bars on the cell windows and inmates have to pay for their own cells by working inside the jail. Available jobs include carpentry, laundry services and even shoe-shining. Drug production inside San Pedro is rife, with some of the purest cocaine in the country made and sold inside. The prison inspired the cult 2003 book Marching Powder by Australian journalist Rusty Young.

His book told the story of Thomas McFadden, a small-time English drug smuggler who was arrested in Bolivia and thrown inside the notorious jail. It also lifted the lid on the drug-trafficking that runs rampant inside San Pedro. One of the photos in this series, taken by photographer Danielle Almeida Pereira, 35, from Sao Paulo, Brazil, shows an inmate handling what appears to be a bag of cocaine. McFadden ended up making a living by giving backpackers tours of the prison, however these were eventually banned when it emerged tourists were also buying cocaine at the 'prison factory'. The Brit was released from San Pedro in 2000 after serving two-thirds of his sentence and now works as a chicken farmer in Tanzania. The lack of guards inside the jail and the fact inmates have to pay for their own cells means San Pedro is a cheap model for a government in one of the poorest nations in South America.

Warren, Rhode Island: Botox could be ‘game changer’ for erectile dysfunction, Canadian urologists say

In a surprising, and seemingly counterintuitive application of a wildly popular wrinkle relaxer, two Canadian urologists are testing Botox for impotence.

If it does for men what it has so far done in aged male rats, Botox may offer a persistent, long-acting (months at a time) way to restore erectile function in men, they believe.

Their preliminary data suggests Botox, or botulinum toxin injections, can increase blood flow to the penis by paralyzing the nerves that cause smooth muscles within the penis to contract.

Erections depend mainly on a good blood flow.

“The advantage of this would be that you inject it once, it lasts for six months potentially, and the pump would be primed every time you wanted to have sex,” said Dr. Sidney Radomski, a professor of surgery and urology at the University of Toronto.

Other forms of penile injection therapy have existed for years, but men have to self-inject every time they want to have sex. Meanwhile, Pfizer’s Blue and other pills that belong to a class known as PDE-5 inhibitors — drugs that act on the chemical signals that open up the blood vessels in the penis — have to be taken before sex, or daily. They can also cause side effects such as headache and heartburn. As well, Pfizer’s Blue doesn’t work for a third or more of men who try it, said Dr. Gerald Brock, professor of surgery at Western University in London.

“It’s probably 50 per cent of men who’ve had prostate cancer surgery; maybe 40 per cent of men with diabetes,” he said. “There are subgroups (of men) where the response rate is almost a 50-50 shot.”

Botox, he said, may help “salvage” those non-responders. Writing in the current issue of the Journal of Sexual Medicine, Brock and co-author, French urologist Francois Giuliano, go further, saying Botox could be a “potential game changer” for ED.

Others worry ED drugs are already perpetuating narrow social norms of masculinity and male sexuality — the idea, says University of Iowa medical anthropologist Emily Wentzell, that “in some way, to be a real man, you have to be this penetrative force.”

She worries categories of sexual dysfunction are proliferating and expanding, and that normal, age-related changes are being framed as “pathological.”

Writing in the Journal of Sex Research, Wentzell argues the medicalization of impotence “and the emphasis on casting phallocentric sex as the natural and healthy sexual ideal have been promoted worldwide through ED drug marketing.”

It’s very much a Euro-North American phenomenon, she said, noting Chinese men, for example, have had a “lukewarm” response to Pfizer’s Blue.

“Obviously medical solutions can help men who feel really bad they can’t live up to their own desires, and maybe their partner’s desires, and certainly cultural scripts about how a man should be,” Wentzell said in an interview.

“But we don’t think about variation in bodily function as natural anymore,” she said. “Instead, we’re narrowing our idea of what counts as ‘normal.’ ”

Men often don’t talk to their partners before seeking out ED drugs she said, or keep their use secret. “Yet research shows that when men talk to their partners, their partners are fine with the change, or they would be fine if they explored other sexual activities.”

Radomski has been using Botox to treat overactive bladders by paralyzing the smooth muscles in the bladder. The injections last six to nine months. He wondered whether it might work in a similar way for penises by allowing the smooth muscles to relax, dilate and allow more blood flow to rapidly fill the penis.

In a rat study, “we saw exactly what he had predicted,” Brock said. “Improved erections were actually seen.”

The animal model isn’t the same as humans. For one thing, the researchers used an electrical current to stimulate the nerves to induce an erection.

While much more research is needed in animals, they hope to start clinical trials in men within six to 12 months. For now there’s little real data, except for one small published pilot study from Egypt. There, a dozen men who received a single injection of Botox showed increased arterial flow and improved scores on a “sexual health inventory” two weeks post-injection.

Still, Botox isn’t innocuous. Made from the bacteria that cause botulism, it can spread to other body areas beyond where it’s injected. In too high doses, “If it goes into the bloodstream, it can kill you, basically, because it can paralyze everything,” Radomski said.

The plan is to use an exceptionally small dose. However, there’s also the potential risk of causing a permanent change like priapism — prolonged erections without sexual stimulation.

“I think that would be a very small risk,” Brock said, “but certainly that would be one of the things that we would be looking for and trying to titrate the dose so it would not occur.”
